Contact your doctor

Make sure that the declaration issued by your company doctor or occupational health doctor includes the following details:

• doctor's name
• doctor's place of work;
• duration of the exemption;
• applicant's name;
• applicant's date of birth;
• doctor's signature.

Completing and sending the request

You will also need to add a few details yourself to complete your request:

• your Schiphol Pass number;
• your vehicle registration number
• Your home adress so we can send you the vignette.
