Temporary medical exemption Contact your doctor

Would you like to request temporary medical exemption? If so, you must submit a request to Schiphol Parking & Mobility Services. You will need a declaration from your company doctor or occupational health doctor, supplemented with a few personal details.

Declaration issued by your company doctor or occupational health doctor

Make sure that the declaration issued by your company doctor or occupational health doctor includes the following details:

  • doctor's name
  • doctor's place of work;
  • duration of the exemption;
  • applicant's name;
  • applicant's date of birth;
  • doctor's signature.

Completing and sending the request

You will also need to add a few details yourself to complete your request:

  • your Schiphol Pass number;
  • your vehicle registration number.
